The Blues suffered another big blow to their hopes of finishing in the top four this season when they were pegged back and held to a 1-1 draw by West Ham United at Stamford Bridge on Sunday.
That result left Chelsea FC in fifth place in the Premier League table and a staggering 10 points behind fourth-placed Tottenham Hotspur with just six games left to play.
The west London side have struggled to find consistent form all season after having won the Premier League title in style under Antonio Conte last term.
Chelsea FC suffered a big blow to their hopes of Champions League qualification when they were beaten 3-1 by Tottenham Hotspur at Stamford Bridge earlier this month.
And Belgian playmaker Hazard has conceded that both he and his team-mates have not been performing to the required level in recent weeks.
Speaking to Chelsea FC’s website after the draw with West Ham, Hazard said: “This season a lot of the games have been the same.
“I remember away at Manchester United, we were in control of the game and in the end we lost.
“Last week against Tottenham, we were in control in the first half and then we lost the game, so this season isn’t going well. We need to work together.”
Chelsea FC will look to get back to winning ways in the Premier League when they travel to Southampton in the top flight on Saturday lunchtime.
